Our free Senior Wellness & Benefits Support Program is designed for older adults in Cuyahoga County who live alone and want to remain independent—but may need extra support staying connected, safe, and financially stable.
Beginning in 2026, Step Forward is expanding this program to provide personalized support, benefits access, and ongoing wellness check-ins for seniors—especially those who are homebound or living with disabilities.
Who can participate?
- Cuyahoga County residents, age 55 or older
- Living alone in a private home or apartment
- Low to moderate income (up to 200% of the federal poverty level)
What you'll receive:
- Daily automated check-in calls to monitor your safety and well-being
- Monthly one-on-one support calls with Step Forward staff
- Help applying for and accessing public benefits and community resources
- Case management support to address individual needs
- Access to mental health counseling services, if needed
- Connections to services that reduce food insecurity and isolation
- Ongoing contact to help you stay safe, supported, and independent
This program is designed to catch small issues before they become big ones—helping you stay healthy, connected, and in your home longer. It also gives peace of mind to you and your loved ones, knowing someone is consistently checking in and ready to help when needed.
